15yr old with pneumothorax

15 yr old male who is a student came to medicine OPD with C/O left sided chest pain since 3 months

HISTORY OF PRESENT ILLNESS:
Patient was apparently asymptomatic 3 months ago then pain started which is
*Dragging type of pain
*Insidious in onset
*Gradually progressive
was treated in a private hospital in his hometown for which he was diagnosed as hydropneumothorax

PAST HISTORY:
*7yrs back Patient developed weakness and pain all over body for which he visited local hospital and was managed conservatively
*4yrs back patient developed similiar complaints of pain all over body and was managed conservatively in our hospital
*2 yrs back he had C/O pain in left side of face and was diagnosed with herpes in local hospital and medication was used for same
*Not K/C/O HTM, DM, TB, ASTHMA, EPILEPSY, CAD

PERSONAL HISTORY:
Diet: mixed
Appetite: normal
Sleep: normal
bowel: regular
bladder: normal micturition

FAMILY HISTORY:
Not significant

GENERAL EXAMINATION:
Patient is C/C/C
-Examined in a well lit room
-Moderately build and nourished
-No pallor, icterus, clubbing, --lymphadenopathy not present

VITALS: 
-Temp: afebrile
-RR: 22/min
-PR: 90bpm

SYSTEMIC EXAMINATION:

CVS: s1s2 +
RS: normal vesicular breath sounds heard
CNS EXAMINATION: NFD

Investigations: 
                       ECG
USG CHEST
CHEST XRAY

        HRCT (done outside)


PROVISIONAL DIAGNOSIS:
Pneumothorax
